All expenses will be evaluated for the event in question in accordance with the information provided in this form. Submission of this form is only a request and DOES NOT CONFIRM the participation of Adam LaVerdiere in said event. Correct information is required to process this request.

I agree that, subject to the needs of the request, each event may require any or all (but not limited to) of the following: deposit, fee, travel expenses, lodging, and meals. In case of confirmation, more details will be provided and included in contract(s), commitment letter(s), and technical rider(s).

I agree that this event will NOT be recorded in audio or video for sale or distribution without the consent and written permission of Adam LaVerdiere.
